COVID LIABILITY

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. SOULBOX Pilates & Yoga adheres to current Government Guidelines to provide a Covid-19 Safe environment.

PROCEDURES FOR ATTENDING SOULBOX Pilates & Yoga DURING COVID 19

I understand that to participate in classes at SOULBOX Pilates & Yoga that I may be subject to some or all of the following before being admitted into the studio space, receive services, or purchase retail items: 

  • Social distancing
  • Proper hand washing hygiene and use of hand sanitizer as needed 
  • Covering of coughing and or sneezing 
  • Being asked to leave if I have a fever, cough symptoms
  • Adhering to current Government guidelines and social distancing measures
  • Pre booking of physical classes, booking must be secured with a valid payment card.  Any no-show or late cancel students will be charged.
  • To adhere to our cleaning protocols we require the yoga studio, changing areas and reception to be clear 10 minutes after the end time of your class.

I understand that the risk that I may become exposed to or infected by COVID-19 is my sole responsibility and SOULBOX Pilates & Yoga waives all liability as aforementioned best practices will be followed. I release all liability from SOULBOX Pilates & Yoga from the actions, omissions, or negligence of myself and others, including, but not limited to, SOULBOX Pilates & Yoga employees, volunteers, and participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the studio or participation in SOULBOX Pilates & Yoga’s events and/or services. 

I hereby release liability and hold harmless SOULBOX Pilates & Yoga, its employees, agents, and representatives from all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of SOULBOX Pilates & Yoga its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any of SOULBOX Pilates & Yoga’s activity or admittance into the facility. 

I understand my responsibility in staying home when ill, informing of SOULBOX Pilates & Yoga of an expected or confirmed case of COVID-19 exposure, maintaining proper hygiene, and following the proper social distancing procedures when participating or attending of SOULBOX Pilates & Yoga.

By creating an account, booking and attending a class I accept  this agreement, I acknowledge the risk associated with COVID-19 and voluntarily assume all risks associated when attending of SOULBOX Pilates & Yoga that may result in personal injury, illness, permanent disability, and death. 

STUDENT DISCLAIMER

I, agree to the following:

  1. That I am participating in the Yoga, Pilates classes or any other exercise programmes offered at SOULBOX Pilates & Yoga during which I will receive information and instruction about yoga, pilates physical exercise, wellness or I recognise that exercise requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
  2. I understand that it is my responsibility to consult a physician prior to and regarding my participation in the yoga, pilates classes, health programs or workshops offered by my instructor or substitute teacher. I represent and warrant that I am physically fit and I have no
    medical condition, which would prevent my full participation in these yoga, pilates classes, health programmes or workshops.
  3. If I am pregnant I understand that I participate fully at my own risk and that of my unborn child/children.
  4. In consideration of being permitted to participate in the yoga, pilates classes, health programmes or workshops, I agree to assume full responsibility for any risks, injuries or programmes offered by my instructor or substitute teacher.
  5. In further consideration of being permitted to participate in the yoga, pilates classes, health programmes or workshops, I knowingly, voluntarily and expressly waive any claim I may have against my instructor or substitute teacher for injury or damages that I may sustain as a result of participating in these programmes.
  6. I understand that from time to time during yoga, pilates classes, the instructor may physically adjust students’ form and posture. If I do not want such physical adjustments, I will so inform the instructor at each class I attend. I also acknowledge that if I do wish to receive such adjustments, it is my responsibility to inform the instructor when an adjustment has gone as far as I desire at that time.
  7. I hereby take full and sole responsibility from any liability of loss or damage to personal property associated with yoga, pilates classes or any other events.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above under my own free will. By booking into any of SOULBOX Pilates & Yoga’s classes or accessing online material I hereby agree to the terms and conditions above.

Upon booking into our classes you accept you indicate acceptance of our waiver.
